What Is Facial Feminization Surgery?

Facial feminization surgery refers to a customized combination of procedures that modify typically masculine facial structures into softer, more traditionally feminine contours. The most significant gender differences in the face are structural — particularly in the forehead, brow, jaw, chin, and nose.

Masculine facial characteristics often include:

  • Prominent brow ridge
  • Lower eyebrow position
  • Wider or angular jawline
  • Square chin
  • Larger nasal bridge
  • Visible thyroid cartilage (Adam’s apple)

Feminine facial features typically demonstrate:

Facial Feminization Procedures Offered

With Dr. James C. Lee, facial feminization surgery may include:

Forehead Contouring & Brow Bone Reduction

Forehead feminization is often the most impactful FFS procedure. Reducing brow prominence and reshaping the frontal bone creates a smoother, softer upper face. This may involve removing part of the forehead bone and setting it back with plates and screws (type 3 – most common), just burring of the forehead bone and orbits, or both depending on each patient’s individual anatomy.

A brow lift may also be performed at the same time to elevate and feminize the brow position.

Hairline Lowering (Hairline Advancement)

Hairline advancement reduces forehead height and creates a more proportionate, feminine facial frame. This is done through an incision at the hairline which becomes less visible over time. If no hairline lowering is desired, the incision for the forehead work can be hidden within the hair-bearing scalp (coronal incision). Either way, patients will experience numbness of the scalp for 12 to 18 months.

Feminizing Rhinoplasty

Rhinoplasty refines nasal projection, narrows the bridge, and softens contours to achieve better harmony with feminized facial features. This may involve shaving down of the nasal bridge, breaking the nasal bones to narrow the nasal width, making the tip of the nose more upturned, and narrowing the size of the nasal tip. Each nasal surgery is customized to the goals of the patient and Dr. Lee takes into consideration the patient’s ethnic identity, skin thickness, airway anatomy, and personal preferences.

Jaw Contouring Surgery

Mandibular angle reduction narrows the lower face and softens a square or angular jawline. This is done through incisions inside the mouth so that the scars are hidden. While this procedure decreases the size and squareness of the jaw, it can also decrease a patient’s jawline definition, especially if the jaw is reduced by a large amount. Dr. Lee helps patients find the fine balance between a more feminine jaw shape and loss of jawline definition.

Chin Reshaping (Genioplasty)

Chin feminization surgery typically reduces the width and projection to create a tapered, refined lower face. This is done through incisions inside the mouth so that the scars are hidden. Chin reshaping can involve cutting excess bone on the chin to create a more tapered, V-shaped chin or cutting the chin into multiple pieces and sliding them into a new position and holding them in place with plates and screws. Each technique has pros and cons – Dr. Lee will recommend the best technique for you and discuss it with you in detail.

Cheek Augmentation

Implants or fat grafting can enhance midface volume, contributing to a heart-shaped, youthful appearance. For implants, this is done through an incision in the mouth and held in place with screws. For fat grafting, this is done through liposuction (usually around the abdomen), purifying the fat, and injecting it into the face. There are pros and cons of each technique and your surgeon will describe them to you in detail. This part of the surgery may not be covered by your insurance plan.

Lip Lift & Lip Augmentation

As we age, our upper lip length increases and the fullness of the lips decreases. Shortening the upper lip can be done through an incision under the nose in order to increase dental show, create a more youthful appearance, and enhance feminine balance. Dr. Lee does this through a “deep plane” technique where the muscle underneath is folded over and secured to the bony structures for a better lift. Lip augmentation can be performed by either using excess skin and fat from the forehead tissue and placing them into the lips or by harvesting fat through liposuction and injecting them into the lips. This part of the surgery may not be covered by your insurance plan.

Tracheal Shave (Adam’s Apple Reduction)

Chondrolaryngoplasty (also known as tracheal shave) reduces the thyroid cartilage prominence for a smoother neck contour. This is most frequently done through a small incision in the neck crease but can sometimes be done through an incision in the mouth using small cameras. The risks of tracheal shave include voice changes, damage to the vocal cords, airway injury, infection, fluid collection, and bleeding so careful consideration must be taken prior to proceeding with tracheal shave surgery.

Recovery After Facial Feminization Surgery

FFS procedures are typically performed under general anesthesia. Depending on the number of procedures performed, surgery may be completed in a single stage or in phases.

Most patients can expect:

  • Swelling and bruising for 2–4 weeks (sometimes the eyelids may swell shut for a day or two)
  • Return to non-strenuous activities within 1–2 weeks
  • Gradual fading and smoothing of the scars over 1 year
  • Recontouring and refinement of the nose and lower face over several months to a year
  • Final results visible as swelling fully resolves after 1 year
